WEDNESDAY, March 21 (HealthDay News) -- Alzheimer's disease and other types of dementia appear to spread through linked nerve cells in the brain, new research indicates.

The findings from MRI brain scans of dementia patients suggest that it may be possible to use MRI to predict the progression of dementias and to monitor the impact of treatments, the University of California, San Francisco (UCSF) researchers said.

They found that Alzheimer's disease and four other common types of dementia appear to move between connected neurons in similar ways, even though they affect different brain networks.

The study appears in the March 22 issue of the journal Neuron.

"Our next goal is to further develop methods to predict disease progression, using these models to create a template for how disease will progress in the brain of an affected individual," study co-leader and neurologist Dr. William Seeley said in a UCSF news release.

"Already this work suggests that if we know the wiring diagram in a healthy brain, we can predict where the disease is going to go next. Once we can predict how the network will change over time, we can predict how the patient's behavior will change over time and we can monitor whether a potential therapy is working," he noted.

In another study published in the same issue of Neuron, a Cornell University team also used MRI to examine brain networks affected by dementia and reported results similar to the UCSF researchers' findings.

Advisory Panel Recommends Approval of NovoTTF for Treatment of Glioblastoma

March 17, 2011 -- An expert panel has advised the FDA to approve a new kind of device to treat recurrent glioblastoma, an aggressive and highly fatal kind of brain tumor.

The FDA is not required to follow the recommendations of its advisory panels, though it commonly does.

The NovoTTF-100A System (NovoTTF) is designed to treat tumors by zapping them with an intermediate-frequency electrical field that's generated by a 6-pound battery pack that a patient carries in a satchel or backpack.

It has been studied for use in patients whose cancer has returned after treatment with standard treatments such as surgery, radiation, and chemotherapy.

The device has two parts: the battery pack and wires that attach to a sheath of electrodes that are attached to the scalp. The rechargeable batteries must be replaced every three hours. And patients are advised to wear the electrodes for at least 18 hours.

The twelve voting members of the Neurological Devices Advisory Panel were asked to vote on three questions: whether or not they considered the device to be safe; whether they considered it to be effective; and whether or not the benefits of using the device outweighed the risks.

The panel voted 12-0 that the device was safe enough for use in a terminally ill population.

On the question of effectiveness, however, the panel was deeply divided, and the vote split with six members voting yes and six voting no.

The chairman of the panel, Robert Hurst, MD, a professor of radiology at the Hospital of the University of Pennsylvania, was called on to break the tie. He voted yes.

In the final vote, on whether the benefits outweighed the risks, the vote was 7 yes, 3 no, with 2 members abstaining.

In explaining her votes, Sarah H. Lisanby, MD, chair of the department of psychiatry and behavioral sciences at Duke University School of Medicine, voiced the concerns of many on the panel.

"I think that the technology this is based on could be a real breakthrough," she says.

Lisanby, like many on the panel, was concerned by the small size and design flaws that likely biased the results of the clinical trial that was presented to determine approval.

The trial included 237 patients who were randomly assigned to receive either the NovoTTF or chemotherapy.

But in analyzing their data, the researchers did not consider patients who could not complete at least four weeks of therapy with the NovoTTF. Some of those were among the sickest patients in the trial, and panel members felt that their exclusion, along with design problems, likely biased the results in favor of the new device.

Overall, the study found that patients survived about as long using the NovoTTF as they did with chemotherapy, with significantly fewer side effects. But many felt that result wasn't definitive.

"There's still a need for more systematic assessment," Lisanby says.

She voted that the device was not proven to be effective and was one of two voters who abstained on the question of whether the benefits outweighed the risks.

"I didn't know how to make a ratio between safety and efficacy when neither was well measured," she says.

Study researchers reported very few side effects while using the NovoTTF. A skin rash, which developed beneath electrodes, was the most common problem and it could be treated with topical steroids.

The panel's decision was delivered to an audience filled with glioblastoma patients, many of whom had traveled to the meeting to observe the deliberations.

"As a GBM [glioblastoma multiforme] patient myself, I'm here because I want to stay alive," says Cheryl Broyles, who has had her tumor come back three times and says she has run out of treatment options.

Scott Johnson, an assistant softball coach from Minden, La., who was diagnosed with glioblastoma in June 2009, when he was 46, told the panel that an initial round of chemotherapy left him feeling exhausted and sick and took him away from his family during what may be his last months.

"When you don't feel good, it's hard to have hope," Johnson says.

In his testimony, Johnson, who said he had traveled to the meeting at his own expense, had urged the panel to approve the device.

"I've worn the device for 14 months, and it has allowed me to continue my life," Johnson says, who was granted access to the device through a clinical trial.

Depending on the size and location of the tumor, glioblastoma may be treated with surgery, radiation, chemotherapy, or a combination.

The Novo TTF surrounds tumors with an intermediate-frequency electrical field that is thought to disrupt cell division, which, over time, may slow or even shrink the tumor.

The technology is approved in some European countries, but is still in late-stage clinical trials in the U.S. for glioblastoma and non-small-cell lung cancer.

"We look forward to working with the FDA to bring this novel, important therapy to patients as soon as possible," says Asaf Danziger, CEO of Novocure, the maker of NovoTTF, in a news release.

FRIDAY, March 23 (HealthDay News) -- Researchers report that they have identified abnormalities in the DNA and RNA of cells in the prefrontal cortex of the brains of autistic children.

The findings may help to explain the underlying mechanism for the brain "overgrowth" that prior reports have documented in autistic children. Those studies have found that the brains of young children with autism are larger than the brains of non-autistic children, particularly in the prefrontal cortex. The prefrontal cortex is key to complex thoughts and behaviors, including language, social behavior and decision-making.

This growth abnormality likely contributes significantly to the social,communication, and emotional deficits common among people with autism, the researchers said.

In the new study, researchers analyzed tissue from the prefrontal cortexes of 33 postmortem brain samples from autistic and non-autistic people aged 2 to 56.

In addition to DNA differences known as copy number variations, researchers also did genome-wide RNA profiling and found differences in RNA between the autistic and non-autistic brains. RNA (ribonucleic acid) plays crucial roles within cells, serving as an intermediary between DNA, the blueprints for genetic information, and the production of proteins that carry out a vast array of vital activities in cells.

The RNA abnormalities appear to be involved with genes that code for proteins regulating cellular growth, the researchers said.

"What we found was the networks that are supposed to regulate the genesis of brain cells and develop them were abnormal. The networks that were supposed to regulate DNA repair were turned down. And the networks supposed to regulate neuron removal and survival were abnormal," said study author Eric Courchesne, director of the Autism Center of Excellence at the University of California, San Diego (UCSD) School of Medicine.

The study is published in the March 22 issue of PLoS Genetics.

How might these differences fit into the autistic picture?

Preliminary research by the UCSD team found that an excessive amount of neurons, or brain cells, might account for the overgrowth. While typically developing kids had about 0.88 billion neurons in the prefrontal cortex, autistic children had about 1.57 billion.

According to the researchers, the copy number variations along with the RNA abnormalities may disrupt the cell cycle and may explain the underlying mechanism driving the overgrowth.

"We found DNA defects, or copy number variations, in a variety of genes that regulate cell production and cell survival," Courchesne said. "To us, that suggests the explanation for why there are an abnormal number of neurons in the prefrontal cortex. Those genes fall into networks that control the number of neurons generated and the number that survive in prenatal life."

Researchers also noted that the RNA differences vary, depending on the age of the brain, with children and adults having different RNA profiles.

Courchesne said that the way the brain responds to that overgrowth of neurons -- in other words, what's happening in those repair pathways -- may help to explain why people with autism may have different trajectories, with some seeming to regress and others continuing to learn new skills throughout their lifetime.

"In adulthood, we see individuals that continue to improve, and continue to gain more and more skills and abilities," Courchesne said. "Then there are others that don't show that continued, ongoing improvement or show the opposite. My best guess is the trajectory has less to do with the original cause of the autism, than with an individual's specific composition of genes, or the available genes to remodel the brain."

Robert Ring, vice president for translational research at Autism Speaks, said researchers offer up a provocative and plausible theory. However, he noted that while brain overgrowth is well-established, only one, small study has shown that the explanation for it is too many neurons.

"What's valuable about the approach this group has taken is that they've gone directly to the tissue of interest, and have asked, 'Is there any evidence there are abnormalities in the expression of genes that correlate with the neuro-anatomic or cellular findings that have been reported?'" Ring said.

"What they're reporting is there is indeed some evidence that particular pathways might be disregulated in the autistic brain vs. the control brain, and some of these pathways, when you look at their function, may be a plausible explanation for the increased growth and increased cell number."

The study, Ring added, offers up new clues for researchers to pursue, but nothing is proven. "There is an enormous amount of work needed to confirm this," he said.
